How We Support You

We offer hourly RN-led care through two focused service options:

Private Duty Nursing

Focused post-hospital recovery support

Designed for individuals transitioning home after surgery, illness, or hospitalization.

Services may include:

  • Post-surgical monitoring and recovery support

  • Dressing changes and incision care

  • Vital sign monitoring and symptom tracking

  • Recovery planning and complication prevention

  • Hands-on nursing support during critical recovery periods

Beginning care with ClearPath is simple and personalized:

Step 1: Consultation Call
Schedule a complimentary 30-minute call to discuss your needs and determine how ClearPath can support you or your loved one.

Step 2: In-Home Meet & Greet
If you choose to move forward, we’ll schedule a 60-minute in-home visit to review your care needs, answer questions, and ensure the right fit before services begin.

Step 3: Care Begins
We initiate customized, RN-led care and ongoing support, tailored specifically to your situation.

Important Information

ClearPath Concierge & Private Duty Nursing LLC is:

  • Not a Medicare or Medicaid home health agency

  • Not a traditional home health agency

  • Not a staffing agency

  • RN-led only — we do not provide non-RN caregiver services

All services are private pay and designed to complement—not replace—your physician and existing care team.

Cancellation Policy

To receive a full refund, cancellations must be made at least 48 hours prior to your scheduled appointment.

Cancellations made less than 48 hours in advance will incur a $50 late cancellation fee.

This policy does not apply to canceled or rescheduled procedures.
