Carefully curated nurse-led care designed to support individuals through two essential services to fill gaps in healthcare.

Hourly support for two nursing care options:

  • Concierge nursing care-proactive wellness care/care management

  • Private Duty Nursing-focused post-hospital recovery.

Step 1: Schedule a 30 min. free consultation call to see how ClearPath Concierge & Private Duty Nursing LLC can assist you or your loved one.

Step 2: If you would like to proceed with services, we will set up a Meet & Greet appointment (up to an hour) to meet in person in your home and to go over more information before proceeding with care.

Step 3: Scheduled care and support begins.

Cancellation Policy

To receive a full refund you must cancel at least 48 hours before your scheduled appointment. Cancellations less than 48 hours before a scheduled appointment will result in a $50 late cancellation fee. Note that the cancellation policy does not apply to canceled or rescheduled procedures.

Concierge Nursing

Hourly, private pay, concierge registered nurse services include:

  • Management of chronic health conditions through weekly, biweekly or monthly wellness checks and coaching

  • Wellness check-ins and health monitoring

    • Vital sign monitoring

    • Assessing for a safe and functional living environment, dietary needs, and activities of daily living to promote independence and quality of life

  • Creation of a personal healthcare blueprint binder to help you, your caregiver and medical team (online version and hard copy)

  • Serving as healthcare advocate and coordinator for the client at appointments or through correspondence with physicians and pharmacists (email, telephone, etc.) with consent

  • Medicine reconciliation, education and resources

Private Duty Nursing

Hourly, private pay, short-term, post-discharge clinical support by a registered nurse to ensure safe transitions home, reduce readmissions, and support recovery with dignity and expertise.

Private-Duty Nursing Services:

  • Meeting client at a hospital/surgery center to listen to discharge instructions and transport client home or meet client at their home to initiate the post-operative recovery instructions/process

  • Post-hospital discharge assessment and dressing changes

  • Wound assessment and dressing changes

  • Injection administration

  • Catheter, ostomy, and tube care

  • Educate home caregivers and family members on the post-operative recovery process to be sure clients understand instructions

  • Medication pickup, organization, and education

  • Helping to obtain medical supplies and equipment when needed

Same Day Services

Examples of same day services include but are not limited to

  • Transportation to and from appointments

  • Medication pickup

  • Wellness checks

  • Communication with medical providers
